Medicare is divided into parts, and each covers something different. Here's what you need to know before picking a plan.
Covers inpatient hospital stays, skilled nursing facility care, hospice, and some home health services. Most people get Part A free at 65.
Covers doctor visits, outpatient services, preventive care, lab tests, and medical equipment. Requires a monthly premium (2025: $185/mo).
A private-plan alternative to Original Medicare that bundles Parts A & B — and usually Part D. Often includes dental, vision, and hearing extras.
Adds prescription drug coverage. Available as a stand-alone plan with Original Medicare or built into most Medicare Advantage plans.
These are the two main paths after getting Parts A and B. Neither is universally better — the right choice depends on your health, budget, and how you use your coverage.
| Feature | Medicare Advantage (Part C) | Medicare Supplement (Medigap) |

| Dental / Vision / Hearing | ✔ Often included | ✘ Not included (purchase separately) |
| Drug Coverage (Part D) | ✔ Usually bundled in | ✘ Need a separate Part D plan |
| Referrals Required | Often yes (HMO plans) | ✘ No referrals needed |
| Travel Coverage | Limited outside service area | ✔ Nationwide + some foreign travel emergency |

Enrolling at the wrong time can mean permanent premium penalties. Here's every window you need to know.
A 7-month window: 3 months before your birthday month, your birthday month, and 3 months after. This is your best opportunity — enroll on time to avoid penalties.
January 1 – March 31 each year. If you missed your IEP, this is your next chance. Coverage starts July 1, and late-enrollment penalties may apply.
If you're still working at 65 and covered by employer insurance, you may qualify for an SEP when you retire. Also available for moves, losing other coverage, and other qualifying events.
October 15 – December 7 — switch Medicare Advantage plans or return to Original Medicare for the following year. Changes take effect January 1.
Part B penalty: 10% added to your premium for every 12-month period you were eligible but didn't enroll — permanently. Part D penalty: 1% of the national base premium per month you go without creditable drug coverage.
